No - in New York State you can see a PT for 10 visits or for 30 days (whichever comes first) without a physician’s prescription. Beyond that we do need a prescription from a physician.

At this time, we are out-of-network with all insurances. We do accept HSA/FSA, and can provide a superbill you can submit on your own for reimbursement if you have out-of-network benefits. Payment is due at time of service. Contact your insurance provider directly to verify the specifics of your out-of-network coverage.
Medicare, Medicaid, No Fault & Workman’s Compensation are not accepted at this time.

My space is yours for the scheduled appointment time. If you need to cancel, please give a minimum of 24hrs notice to avoid a late cancellation fee of $70. If you do not show up for your appointment at all and have not contacted us, you will be charged the full fee of the appointment.
We do understand that life happens, and may choose to waive the fee in extenuating circumstances.

I will take a comprehensive history of your complaints to identify possible causes for your symptoms. We will then do a range of motion assessment, strength and neurologic assessment as needed. From there, I will identify areas of motion limitation to begin to build a personalized program to control your symptoms. Hands on work is generally used to facilitate improved motion and reduced muscle spasm. Every visit is a reassessment to adapt your program to your needs
